You have been asked to read and sign this form so that United Jewish Appeal of Greater Toronto and its affiliated organizations and programs (“UJA”) will be permitted to use your video (“Video”), images from the Video, name and any other information that you have provided to UJA in connection with the Video (subject to the restrictions below). For the purposes of this consent form “UJA” shall include: The Jewish Foundation of Greater Toronto, UJA Federation of Greater Toronto, and their programs.
We ask you to sign this form to acknowledge your consent for UJA’s use of the Video, images from the Video, name and any other personal information which you included in connection with the Video. By signing this form, you grant UJA a license to use the Video, images from the Video and other information in perpetuity, as further outlined in this consent form herein.
Thank you very much for taking the time to prepare the Video for our use. Through the Video and effort, we hope to strengthen our community - to advance UJA’s programs and fundraising campaigns, and engage other community members in our programs and UJA’s philanthropic opportunities.
Consent: I hereby acknowledge as follows:
- I give UJA the absolute right to and/or permission to publish my video (in writing, photo, and video) in the following manners: on UJA websites, affiliated sites, and blogs; on UJA-owned and managed social media websites and social media ads; and In UJA produced video.
- I give permission for UJA to use my first name.
- I hereby waive any right that I may have to inspect and/or approve the finished product that may be used in connection therewith, or the use to which it may be applied.
